CONSULTATIONS AND REFERRALS

Consultations

Prior to your first consultation please complete an owner consent and pre-treatment form, to speed up the process, this can be found by following the link below. I also require your veterinarian to complete a referral form, if you could please send them a copy and request that the completed form be emailed to Hurdles. It is a legal requirement for Hurdles Veterinary Physiotherapy to gain veterinary consent from your registered veterinary practice, as a requirement under the Veterinary Surgeon’s Act 1966, and the Veterinary Surgery (Exemptions) Order 2015.

These forms provide the background information needed to carry out an assessment and treat all patients.

Once I have these forms, you can book a consultation immediately.

Please email all forms to Kirsten@hurdlesvp.co.uk

Thank you

Initial Consultation

The first consultation requires a little more time to get to know you and your pet, it takes around 90 minutes. We will go through their routine and lifestyle, assess them whilst still and moving and take on board what you wish to get out of your sessions. The session will also include the formation of a treatment and exercise program.

Routine Consultation

After the initial consultation, you will only need routine consults, as long as it is within 6 months of a previous session and they have not since been diagnosed with a new condition or acquired a new injury. Routine consultations are exactly the same, but the assessment is not as thorough, as I only need to see if anything has improved since last time or if any lameness has occurred. The sessions also allow for a longer treatment time for maximum benefits.
